The definition of a split/shared visit has been a topic of significant discussion over the past several years. CMS/Medicare had specific points that the CPT Guidelines did not address, such as whether the two practitioners needed to be in the same group, if the billing provider had to perform the substantive portion of the visit, and what locations split/shared visits were allowed. There were separate rules or guidelines for these services between a physician and a nurse practitioner or physician assistant.
In 2024, CPT redefined split/shared visits with clearer guidelines and CMS/Medicare announced that they would follow these updated CPT Guidelines. Additionally, CMS/Medicare mandated the use of the FT modifier for all these visits. Recently, CMS/Medicare provided the Medicare Administrative Contractors with additional guidance on these visits, which may prompt you to re-evaluate how your practice performs and documents these shared visits.
